The Transitions in Care (TiC) initiative is co-led by the Institute of Circulatory and Respiratory Health (ICRH), Institute of Health Services and Policy Research (IHSPR), Institute of Human Development, Child and Youth Health (IHDCYH), and the Institute of Indigenous Peoples’ Health (IIPH), in collaboration with the Institute of Aging (IA), Institute of Cancer Research (ICR), Institute of Gender and Health (IGH), Institute of Musculoskeletal Health and Arthritis (IMHA) and Institute of Neurosciences, Mental Health and Addiction (INMHA), and in partnership with Mitacs and The Rossy Foundation.

Overview

The focus of Transitions in Care (TiC) Team Grants phase 2 is to support inter-disciplinary teams that are poised and ready to optimize health outcomes during transitions in care. The grants will provide support to develop, implement, evaluate, and equitably spread and/or scale (share) pragmatic solutions to challenges associated with transitions in care from a clinical, health care or health system perspective. This funding opportunity welcomes any clinical, health systems, health services, policy, community-based, and/or digital health innovation research related applications addressing applied research questions deemed relevant and important by health system decision-makers in Canada. The proposed research should be relevant to communities and decision-makers by engaging them in the research process and producing results that can be applied and/or adapted to multiple regions and/or settings. In addition, the proposed TiC solution(s) should have the potential to improve the health outcomes and experience of all peoples within Canada including Indigenous Peoples (First Nations, Inuit, Métis and/or Urban Indigenous communities), racialized groups and historically underserved and/or excluded groups, undergoing transitions in care.

Objectives

The TiC team grants are expected to:

• Produce results that are applicable, adaptable and ready for equitable spread and/or scale (sharing) by a healthcare or community care provider through implementation research that is relevant to clients/patients/families, communities, and decision-makers;
• Improve health outcomes, client/patient/families experience, effectiveness, and lower the cost of care within two or more of the TiC focus areas through identifying, evaluating and implementing evidence-based and/or effective interventions;
• Build novel multi-disciplinary and trans-disciplinary collaborations and partnerships with individuals, communities and organizations in Canada that have interest in transformative research in care transitions by promoting engagement, recognition and respect for health research expertise across an array of health disciplines, research pillars, and industry; and,
• Build and promote relevant capacity to conduct research in transitions in care, including training and early career development by supporting teams that actively engage established researchers, new investigators, trainees, and knowledge users.

Research Areas

This funding opportunity will fund inter-disciplinary teams to address the intersection of two (2) or more TiC Focus Areas:

• Across the Life Cycle;
• Changing Health Status or Care;
• Key Populations to Optimize Transition in Care Outcomes.

Within the scope of this funding opportunity, in addition to being centered on the intersection of two or more TiC Focus Areas, this funding opportunity will also support the following funding pools:

• Indigenous Health Research.
• Care transitions for older adults facilitated by digital health technologies.
• Mental Health Care for Post-Secondary Students.
• Primary Care and Community Mental Health Care for Youth.
• Mental Health Care for Black Youth.

Funds available

• The total amount available for this funding opportunity is $11,520,000 enough to fund approximately twelve (12) grants. This amount may increase if additional funding partners participate.
• Of this $11,520,000:
  • $6,720,000 is available to fund seven (7) grants focused on the intersection of two or more TiC focus areas.
  • $960,000 is available to fund one (1) grant specifically focused on Indigenous Health Research.
  • $960,000 is available to fund one (1) grant specifically focused on care transitions for older adults facilitated by digital health technologies.
  • $960,000 is available to fund one (1) specifically focused on Mental Health Care for Post-Secondary Students.
  • $960,000 is available to fund one (1) grant specifically focused on Primary Care and Community Mental Health Care for Youth.
  • $960,000 is available to fund one (1) specifically focused on Mental Health Care for Black Youth.
• The maximum CIHR amount per grant is $240,000 per year for up to four (4) years for a total of $960,000 per grant.

Applicants are required to have a health system partner. Applicants are also strongly encouraged to seek additional in-kind and/or cash contributions from other partner organizations. Partner organizations include but are not limited to universities, foundations, voluntary organizations, health charities, hospitals, community groups and/or organizations, Indigenous organizations, health provider associations, government departments, and organizations in the private sector. Note that federal sources of funds will be accepted within the context of activities that engage Indigenous communities/organizations.
